Student Disability Services Specific Disabilities Guidelines

Student Disability Services has developed guidelines and verification forms to assist students in gathering the information needed for certain specific disabilities. These guidelines and verification forms are intended to facilitate the gathering of the information needed by the ADA Committee in its review. The guidelines and verification forms should not be interpreted as a restriction on the type or quality of information that the ADA Committee may require. The Committee may, for example, need updated or additional information regarding a request or disability, even though the updated or additional information is not mentioned in the guidelines or verification form.
